What is included in the cost of care?
Home2Me is a private‑pay personal care home; however, certain long‑term care or life insurance policies may provide reimbursement when applicable. Our monthly rate includes room and board, personal care services, meals, housekeeping, laundry, and routine facility services. Additional personal hygiene products, personal items, and any durable medical equipment (DME) recommended by a healthcare professional are the responsibility of the resident or family.
Do the rooms have private bathrooms?
Our facility offers 16 resident rooms. Eight rooms feature Jack-and-Jill-style bathrooms, while the remaining rooms have access to shared common bathrooms equipped with two handicap-accessible stalls and a walk-in shower. Bathroom accessibility is carefully managed, and with occupational therapy support available, accommodations are made to ensure resident safety and comfort.
Do you offer activities and social programs?
We provide a variety of engaging activities designed to promote socialization, enjoyment, and overall well-being. Family members and volunteers frequently participate in hosting activities and special events.
Is there a nurse or doctor on-site at all times?
Our in-house physician performs routine monthly visits and is available as needed. Licensed nurses are regularly present in the facility, with additional nursing support available on call.
Can you accommodate special dietary or feeding needs, such as diabetic diets?
Our trained staff carefully follow physician-ordered dietary restrictions and individualized nutrition plans.
What kind of meals do you serve?
We prepare homemade meals and desserts with an emphasis on fresh, locally-sourced ingredients whenever possible.
Can residents have a TV or phone in their room?
The facility provides free Wi-Fi throughout the building, along with cable television and phone access in common areas. Residents may arrange private in-room cable or phone service directly through local providers if desired for a nominal fee.
Can residents bring their own furniture?
While all rooms come fully furnished, residents are welcome to bring personal furniture and belongings to make their space feel more like home.
Can family pets visit?
Pets are welcome to visit with appropriate vaccination and health documentation.
Will residents receive assistance with bathing and dressing?
Staff provide assistance with personal care needs while encouraging independence whenever possible to promote dignity and safety.
Do you have your own doctor and pharmacy?
We offer access to an in-house physician and pharmacy for convenience. Residents are also welcome to continue using their preferred healthcare providers and pharmacy.
Do you provide transportation to appointments?
While we do not currently offer facility transportation, we assist in scheduling appointments and can help coordinate local transportation services for a nominal fee.
What happens if my loved one’s care needs increase?
Residents do not automatically need to leave if their care needs change. We work closely with outside agencies and healthcare providers to accommodate increased needs whenever possible. If a resident’s medical condition becomes too complex for our level of care or presents safety concerns, we will work with families to identify appropriate next steps.
Do you offer therapy services?
We partner with outside agencies that provide occupational, physical, and speech therapy services within the facility. Our staff works closely with therapy teams to ensure continuity of care.
Do you provide end-of-life care?
We work closely with hospice providers to ensure residents receive compassionate end-of-life care in a peaceful, comfortable environment.
How are medications managed?
Medication reminders and administration are provided according to each resident’s individualized care plan and physician orders with ongoing monitoring.
Do you create individualized care plans?
Every resident receives a personalized care plan tailored to their needs, preferences, and level of support required. Care plans are regularly reviewed and updated as needed to ensure each resident continues to receive appropriate, high-quality care.
Do you have emergency call buttons?
Emergency call systems are available to ensure residents can quickly alert staff when assistance is needed.
Is staff available 24/7?
Trained staff are present at all hours to provide supervision, assistance, and support.
How do you assess resident needs before move-in?
Prior to admission, we conduct a comprehensive assessment to evaluate each individual’s physical, medical, and personal care needs. This helps ensure Home2Me is the right fit and allows us to develop an appropriate care plan before move-in.
